WAIVER AND RELEASE OF LIABILITY
This Waiver and Release of Liability ("Agreement") is entered into by the registrant and/or their parent or legal guardian if the student is under eighteen (18) years of age, in connection with participation in all training sessions, grading ceremonies, competitions, events, and activities organised by or affiliated with World Taekwondo Mandeville ("WTM"), operating at the Church Teachers' College Gymnasium, 40 Manchester Road, Mandeville, Jamaica.
1. Acknowledgement of Risk
I understand and acknowledge that taekwondo and martial arts training involves physical activity and inherent risks of injury, including but not limited to bruises, sprains, fractures, dislocations, concussions, and in rare circumstances, serious or permanent injury or death. I am fully aware of these risks and voluntarily assume all such risks on behalf of myself and/or the minor student for whom I am registering.
2. Release & Indemnification
In consideration of being permitted to participate in WTM activities, I hereby release, waive, discharge, covenant not to sue, and hold harmless World Taekwondo Mandeville, World Taekwondo Jamaica (WTJ) and Church Teachers' College (collectively, the "Released Parties") from any and all liability, claims, demands, causes of action, or suits arising from participation in any WTM activity — whether caused by the negligence of the Released Parties or otherwise.
I further agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, cost, or expense that they may incur due to my or my student's participation in WTM activities.
3. Medical Treatment Authorisation
In the event of a medical emergency in which I cannot be reached in time, I hereby authorise the Released Parties to seek and consent to emergency medical treatment on behalf of the student. I accept full financial responsibility for any and all medical expenses incurred. I confirm that the student is in good physical health and fit to participate in martial arts training, and have disclosed all known medical conditions, allergies, and medications in this registration form.
4. Code of Conduct & Discipline
I understand and agree that all students, parents, and guardians are expected to uphold respect, discipline, courtesy, and sportsmanship at all times — both on and off the mat. WTM reserves the right to suspend or permanently dismiss any student for misconduct without refund. Conduct that is disruptive, disrespectful, or harmful to others will not be tolerated.
5. Fees & Payment Policy
A registration fee of JMD $1,000.00 is payable upon enrolment. I understand that World Taekwondo Mandeville will give 30 days' written notice of any fee increase. Monthly tuition fees are non-refundable after payment has been received. Failure to pay fees may result in suspension of training until the outstanding balance is settled.
6. Grading & Rank Testing
I understand that rank advancement at WTM follows the Kukkiwon standard system. Promotion testing is held every three (3) months, provided all requirements — minimum training time, class attendance, nine stripes earned, and instructor recommendation — have been met. Rank is earned at the individual's pace and is not guaranteed within any fixed timeline.
7. Photo & Media Consent
From time to time, World Taekwondo Mandeville will photograph and video-record students during training, grading events, and competitions. These may be used on the WTM website, social media platforms, and promotional materials. If you do not consent to your or your child's image being used, please notify WTM in writing at worldtaekwondomandeville@gmail.com.
8. Privacy & Data
Personal information collected during registration is used solely for administrative and communication purposes. WTM will not sell, rent, or share your data with third parties without your explicit consent, except where required by law.
9. Communication & Notifications
I understand and agree that timely and open communication is a fundamental pillar of the World Taekwondo Mandeville community. I commit to maintaining an accurate, up-to-date email address and phone number with WTM at all times and to responding to official communications — including fee notices, schedule changes, grading announcements, and emergency alerts — within a reasonable timeframe. WTM will use the contact information provided in this form as the primary means of reaching the student and/or parent/guardian. I further acknowledge that failure to respond to critical communications (e.g., unpaid fees, missed grading windows) may result in temporary suspension of training until the matter is resolved. I accept responsibility for keeping WTM informed of any changes to my contact details.
10. Participation & Attendance
I understand that consistent participation and regular attendance are essential to a student's progress, safety, and the overall quality of the programme. I commit to notifying WTM in advance — where possible — of planned absences, and I acknowledge that extended or unannounced absences may affect eligibility for grading, team selection, and competition opportunities. I further agree that:
(a) students are expected to attend class prepared, in the appropriate dobok, and with a respectful attitude;
(b) parents and guardians of minor students are encouraged to actively support their child's attendance and engagement;
(c) any recurring absence without communication may be treated as a voluntary withdrawal from the programme, subject to the refund policy outlined in Clause 5 above;
(d) participation in special events, tournaments, and seminars is encouraged but voluntary unless otherwise stipulated by the instructor.
11. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of Jamaica. Any disputes arising under this Agreement shall be subject to the exclusive jurisdiction of the courts of Jamaica.
