04 Waiver and agreement

1 · Acknowledgement of risk

Taekwondo is a full-contact martial art. Training, sparring, breaking and conditioning carry an inherent risk of physical injury — including bruising, sprains, fractures, dental injury and concussion — that cannot be eliminated by supervision, protective equipment or instruction. I voluntarily assume those risks on behalf of the student named above.

2 · Release and indemnification

In consideration of the student being permitted to participate, I release and agree not to sue World Taekwondo Mandeville, its instructors, officers and volunteers from liability for injury or loss arising from participation in classes, gradings, demonstrations and events, whether on or off the training premises, except where caused by gross negligence or wilful misconduct. I agree to indemnify them against any claim brought on the student's behalf.

3 · Medical authorisation

I confirm the student is medically fit to train and that I have disclosed every condition, allergy, injury and medication relevant to safe participation. In an emergency where I cannot be reached, I authorise the instructor to secure necessary medical treatment, and I accept responsibility for its cost.

4 · Conduct, fees and grading

The student will observe the courtesies and discipline of the dojang. Monthly tuition is payable in advance and is not pro-rated for missed classes; fee amounts and any changes are communicated in writing. Rank progression follows the Kukkiwon grading system; eligibility to test is determined by the instructor, and testing fees are not refundable on an unsuccessful attempt.

5 · Communication

Routine notices — fees, schedule changes, grading announcements and emergency alerts — are sent by WhatsApp to the number given in this registration. I will keep my contact details current and respond to critical communications in good time.

6 · Photos and media

Photographs and video are taken from time to time at classes, gradings and events. Consent to their use in school publicity is optional, given or withheld below, and has no effect on participation. It may be withdrawn in writing at any time.

7 · Governing law

This agreement is governed by the laws of Jamaica. By typing your name as signature below, you confirm you have read, understood and voluntarily agree to these terms. For students under 18, a parent or legal guardian must complete and sign this form.
