I/We acknowledge that participation in Taekwondo at World Taekwondo Mandeville involves physical activity with inherent risk of injury.

I/We voluntarily accept all risks including physical injury, disability, or harm from training activities including sparring, breaking, and competition.

I/We agree to follow all instructor instructions and abide by WTM rules. All instructors hold certifications under World Taekwondo (WT) and Kukkiwon standards.

Monthly dues are due by the 1st of each month. A registration fee of JMD$1,000 is due at the first class.

I/We give permission for photos/videos taken during training to be used for WTM promotional purposes unless a written request is submitted otherwise.

By submitting this form you confirm all information provided is accurate and complete.
